Generate Acceptance Form

This form will allow you to generate an Acceptance Form tied to a checkbox field.

Title Enter agreement title displayed to the user. Sub-title Enter agreement title displayed to the user located below the title. Message Enter agreement message displayed to the user. Options Enter options for Acceptance form.
